Use the link below to download the "Atax Effectiveness Measurement Form" to report uses of the organization's prior Atax grant, if applicable.

• When completing the form, reference the Categories in the Descripton of Operations of this application, and report on the specifics of each Category awarded to your organization in its most recent Atax grant.
• If you choose to create your own format for the Executive Summary instead of using the Atax Effectiveness form, please reference the form and use the criteria as a guideline.
• Organizations will have the opportunity to upload the Atax Effectiveness Measurement Form as part of their application or provide the appropriate information in a text area provided in the Executive Summary section of the application.
• Please note that there is an two page limit for the executive summary.
